Hi, normally, when I am for instance in a shop and someone says something like “bom fim de semana!” I normally reply “obrigado igualmente!”. But I notice here the patient replies “A si também”. Would it be appropriate to use this instead of my normal reply?

Here, the “a si também” is specifically in response to “é bom vê-la”. The full sentence would be “É bom vê-lo a si também”. In other contexts, in response to a “have a nice day” or similar, you should use “para si também” (different preposition). That is perfectly appropriate, just as much as “Obrigado, igualmente!”, so feel free to use either one freely 🙂
